Job Requirements

• BLS (AHA or ARC only) required for onboarding
• FMOLHS Skills Checklist within one year prior to start date; updated annually
• FMOLHS Core Competencies current within a year prior to start with passing score of 80% or higher; updated annually
• Proof of 2 MMR immunizations, positive titer or declination accepted
• Fit Test (N95 mask types) completed within 1 year prior to start; updated annually
• Proof of 3 Hepatitis B immunizations, positive titer or declination accepted
• Influenza Vaccine required during flu season (10/1 to 4/30); medical/religious exemptions accepted with proper documentation
• TB Test: Negative QuantiFERON Blood Test within one year prior to start OR 2-Step TB Skin Test within specified timeframes; if positive, MD note or proof of treatment required
• Health Statement signed and dated within 1 year of start stating ability to work without restrictions
• Proof of 2 Varicella immunizations, positive titer or declination accepted
• Proof of Tdap vaccination within past 10 years or declination accepted
• Annual TB Questionnaire required within 6 months of start; documentation required if positive
• FMOLHS Criminal Background Check within 30 days prior to start including SSN Address Trace, county and national checks, education and employment verification, driving record
• FMOLHS Drug Screen (Negative 10 Panel) within 30 days prior to start
• FMOLHS Competency Exam within year of start with passing score of 80% or higher
• FMOLHS Badge Photo (forward facing, well lit, neutral background)
• FMOLHS Contract Annual Education Packet
• FMOLHS Eye Exam & Color Vision (Ishihara Color Test and Snellen Vision test) within 1 year prior to start
