What you can expect:
Reporting to the Director of Healthcare Services/Administrator. The Case Manager will be accountable for making patient care driven decisions based on your educational preparation and experience in nursing. You will supervise all direct patient care staff for your case load.
How you will make an impact:
- Provides services that are ordered by the physician as indicated in the plan of care
- Regularly assesses changes in the patients status since the last visit/treatment day, and determines whether the planned services should be modified,
- Performing a re-evaluation and obtaining revision orders from the physician as indicated.
- Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
- Participates in Quality Assessment and Performance Improvement (QAPI) program.
- Supervises professional and paraprofessional staff, as assigned
Minimum Education & Experience Requirements:
* Current license as a Registered Nurse in the State of Illinois
Knowledge, Skills & Abilities Required:
- Current CPR certification
- Valid driver's license and auto liability insurance
- Able to travel within geographic area serviced by office from assignment to assignment.
- Must have phlebotomy and IV experience
ADA Specifications:
- Must be comfortable visiting patient's in home
- Occasional exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
- Able to constantly stand, walk, bend, stoop, squat, kneel and reach freely.
- Able to frequently lift, reposition, and transfer patients.
