Summary: St. Anthony Hospital's Radiation Oncology team is looking for a locum provider to help with coverage gaps Wed-Fri, starting Jan 22 through May 2. Shift includes 8a-5:30p Wed-Friday.
BID DUE DATE: Dec 6th at 5p EST!
Rate Caps for Submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"
Hourly - *** and below - "Per hour, not OT"
Holiday - Must be 1.5x the regular rate*** "Per holiday hour, no OT"

Wednesday-Friday - 8:00a-5:30p
Facility Location: 11750 W 2nd Pl, Suite 150, Lakewood, CO 80228
Setting: Outpatient
FTE: 0.675
Days per month: 3 shifts per week, about 12 shifts per month, dependent on the number of weeks per month.
Required Procedures: Radiation Oncology, Consultations/New Patient Visits, Follow ups, Established Patient Visits, On Treatment visits, Treatment Planning, Treatment Supervision.
EMR: EPIC, Aria
Machine is a Varian True Beam. Treatment planning is done in Eclipse.
Support Staff: RN
Reason for Coverage: Gaps in coverage.

EMR Required Training:
Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training.
EMR Virtual training - Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.

Air Travel: Local, drive in candidates only.

Credentialing Information:
Timeframe: 60-90 days is standard.
Emergency temp privs? Yes with clean file only. - 1 week minimum needed to complete file.
Credentialing with other facilities? No
Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No

Admissions - 25-35 Treatments.
Is Cyber Knife or Brachytherapy required? No, not at this location.
What is the make-up of your support staff? 2 Radiation Therapists, 1 CT Sim Radiation Therapist, 2 RNs (1 on Monday and Friday), 1 PSR (Registration/Scheduling)
I see 1 FTE is being asked - Willing to use 2-3 providers Preference will be given to single providers who are able to cover a majority of the dates.
SUBMISSION REQUIREMENTS
Board Certification Radiation Oncology - Required
Active CO license - Required or IMLC (LOQ in hand at time of submission)
Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
Local, drive in candidates only - Required
Site willing to accept 2 providers to fill the need, candidates must be available Wed-Fri for the weeks available. Candidate must confirm wed-fri availability and state the specific weeks they are able to offer between the dates need - Required, candidates who are able to cover the full stint every week will get priority over those not able to fill all the weeks.
Clean MP/background - Highly Preferred, site will place priority for candidates with a self inquiry NPDB at time of presentation.
