•\tPay: $100 Hr.
•\tCoverage: ASAP – 6 months
•\tSchedule: Monday - Friday, 8:30-5:00pm.
•\tSetting: Clinic
•\tDaily Responsibilities: Provide Population Health via Virtual and in Person coverage, Average 4 days weekly virtual work in clinic, 1 day in person, Preventative Care, Diabetes, Hypertension, Blood Pressure, Medicine Management, Women’s Health/Paps, Asthma.
•\tNo Call
•\tPatients per shift: Estimated 24 Patients per day (20 Minute Video and 10 Minute Telephone Appointments), In Person: Estimated 20 Patients per day
•\tEMR: Epic
•\tCredentialing timeframe: 12 Weeks from time application submitted
Requirements:
•\tActive unrestricted CA License
•\tActive BLS certifications
•\tMinimum 3 years of experience
•\tBoard Certified
•\tVaccination Requirement: Will consider waivers
•\tFull-time coverage is mandatory—no deviations from the requested schedule will be accepted.
•\tMust be able to provide at least 2 supervisory references.
