Job Description
Provides supportive and educational services to youth and adults with Intellectual/Developmental Disabilities who have the Home and Community Based waiver and/or Medicaid, including Medicaid MCOs. The Community Support Worker will provide support to individuals in their homes and the community by teaching skills that foster independence and social connections and promote mental health. Services are primarily offered in the northern part of Chicago and the northern suburbs.
Knowledge, Skills and Responsibilities:
Requirements:
Must be at least 21 years old, with a high school diploma or GED.
Demonstrated ability to accept supervision and work constructively with individuals, colleagues and the community
Direct Support Professional (DSP) certified preferred, or ability to complete training to become certified within 60 days of hire.
Ability to work a flexible yet consistent schedule (evenings and weekends required)
Complete CPR/First Aid training.
Complete Physical and Background Check.
Must have access to a dependable car, valid Illinois Driver’s License, and auto insurance.
Must be able to meet physical demands of the position including but not limited to standing and walking.

Compensation: The compensation for this role is $20.51 per hour. Plus, mileage reimbursement
